About this role
As a Personal Care Coordinator in Specialized Care, you provide individualized support to residents by coordinating their care according to their care plans. You ensure care is delivered carefully and consistently, serve as the main contact for families and stakeholders, and contribute to creating a safe and warm living environment. You work either at Eckartdal, a residential park in Eindhoven with gardens and community spaces, or in smaller community homes supporting clients with mild intellectual disabilities toward greater independence and social participation.
