About this role
Personal care coordinator for specialized residential care at Woonpark De Donksbergen in Duizel. You provide individualized support to residents with complex care needs, including behavioral regulation and intensive nursing care. You coordinate daily care activities, work closely with multidisciplinary teams (behavioral specialists, speech therapists, doctors), and serve as a trusted point of contact for residents, families, and colleagues.
